If you are using the Golan driver and your server has the IOMMU enabled in UEFI. There's usually a setting somewhere to disable the IOMMU, but it's often quite well hidden and obscurely named. See https://github.com/ipxe/ipxe/issues/1585#issuecomment-3743827488
